P tion Responsibilities: xc2xb7 ions as an advanced practitioner within primary care xc2xb7 Pr es consultation on nursing practice and assigned patient outcome. xc2xb7 In aboration with physician, orders and interprets diagnostic tests. xc2xb7 Revi all admission data within the designated time frames; determines suitability of the level of care in accordance with hospital established criteria. xc2xb7 Procu services and serves as advocate on behalf of patients, families and the hospital for scarce resources throughout the continuum of care. xc2xb7 Facili s and provides necessary patient and family education on admission as indicated. xc2xb7 Partici s in the inter-disciplinary treatment planning conferences. Provides guidance to team when doing patient care conference. xc2xb7 Demonstr advance knowledge of the general diagnostic and therapeutic procedures and nursing care of patients on the unit. xc2xb7 Interview tients and completes a full comprehensive medical history and physical examination within 24 hours of admission. xc2xb7 Documents information relative to the patient’s history, physical examination, assessment and treatment plan following unit policy and standards. xc2xb7 Initiates r rals & collaborates with medical staff and other health care professionals.
