Who We Are:We are a nephrologist-led renal care company helping patients in post-acute settings recover and return home after their hospital stay. We operate primarily in skilled nursing facilities (SNFs), working as an extension of the in-house nursing team and in collaboration with other consultants including cardiology and pulmonology. We aim to offer our partner homes a team-based, renal-focused layer of clinical oversight to ensure patients do not experience preventable renal complications. With nephrology support, we reduce burdens on in-house nursing staff while allowing homes to be better equipped to care for patients requiring renal care.
