Working as a member of the inter-professional team, the Nurse Practitioner (NP) will provide specialized services in the management of patient care & safety. The NP functions in an expanded role demonstrating a high level of autonomy and is responsible for: conducting patient assessments, diagnosing; ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice.xc2xa0Responsibilities/Duties:Provides comprehensive primary care to the patients as part of an interdisciplinary/inter professional team focused on creation of a person-centered plan of care.xc2xa0Engages clients in risk factor and health problem identification, goal setting, and making choices of intervention for disease treatment, prevention, and health promotion.Experience with screening for chronic disease or substance abuse disorders.xc2xa0Orders and interprets appropriate screening, diagnostic, and laboratory tests.xc2xa0Keeps complete and accurate records of client visits through timely documentation.Uses and adapts assessment tools and techniques based on the clientxe2x80x99s unique needs.Reviews and updates primary care activities in line with organization goals and projected outcomes.Promotes evidence based practice through the use of latest research based guidelines.Ensures the provision of safe, evidence based, patient and family centered care.Keeps up to date with health related policies and implements if deemed appropriate to impact patient care in a positive manner.Participates in ongoing quality monitoring and improvement.Participates in creating, delivering and maintaining an exceptional quality program.Creates an environment that builds on the principles of integrated team approach.Accountabilities:Demonstrated ability to collaborate and communicate effectively in a team setting to develop appropriate, patient centered, individualized care plans.Ability to maintain effective and professional relationships with patients and other members of the care team.Supports a respectful workplace and ability to work in a collaborative practice model.Contributes to the development of learning resources and initiates health education and other activities that assist, promote, and support patients.xc2xa0Strong health assessment and clinical skills.Ability to prioritize, manage time effectively, and be flexible within an active work environment.Self-motivated.Considers the psychosocial, emotional, ethnic, cultural, and spiritual dimensions of a clientxe2x80x99s health without judgement.Working knowledge of differential diagnosis of chronic diseases or substance abuse disorders, when appropriate.xc2xa0Ability to effectively engage patients in a therapeutic relationship, when appropriate.xc2xa0Ability to work with patients by telephone as well as in person.xc2xa0Excellent leadership, critical thinking, organizational, and communication skillsAbility to report data trends for the quality improvement process.xc2xa0Comfortable with working within a collaborative, multidisciplinary care team.Demonstrates organizational and communication skillsQualifications:Masters in Nursingxc2xa0Must have current registration with the College and Association of Registered Nurses of Alberta (CARNA)xc2xa03-5 yearsxe2x80x99 nursing experienceA Criminal Record and Vulnerable Sector check.xc2xa0The MacEwan University Health Centre offers a competitive compensation package, comprehensive benefits package, and opportunities for continued education.
