We are looking for Nurse Practitioners to join our team in conducting patient centered concierge level care in Medicare patients’ homesThe NursePractitioner makes up our patient intake team and are dedicated to onboardingeligible patients. The Nurse Practitionersare focused on completing the initial visit and assessment that begins thepatientxe2x80x99s longitudinal care with WellBexe2x80x99s broader care team. In this role the Nurse Practitioner enganges the members and delivers primary care in the home-based settingfor patients. The Engagement Nurse Practitioners is a critical member of thecare team that will be accountable for not only engaging new patients, but alsoensuring we have a comprehensive medical record on file for the patient bycompletingxc2xa0 accurate, timely documentation of the patient records forthese visits as part of ourxc2xa0model that will support transitioning these new patients to our longitudinal care Nurse Practitioners.xc2xa0Competencies:Performs initial patient screenings/physical examinations/creates assessments plansOrders, interprets, and analyzes diagnostic tests and determines the need for follow-up appointments and further treatment options.xc2xa0Delivers quality care while maintaining all company metrics pertaining to appropriate documentation.coding, HEDIS measure, engagement of patients and managing medical expense.xc2xa0Qualifications:Experience in family practice, geriatrics, hospice care, palliative care, hospitalist, ICU, SICU, ERxc2xa0Minimum Master’s Degree in Nursingxc2xa0Valid nurse practitioner or physician assistant license/Board Certificationxc2xa0NPI number as an NPBLS/ACLSValid DL, Auto Insurance, willingness to drive to patients homes/patient locationsalary $103,000 – $137,000xc2xa0 Plus bonus and full benefit package 4 weeks PTO amd moreNo covid vaccinations required
