New Grads NP’s are welcome to apply.We are looking for a Full Time Nurse Practitioner Telehealth that is able to work 5 shifts per week which includes 3-4 weekend shifts per month.Job Summary:The Nurse Practitioner will be overseeing patient-care operations, monitoring, identifying and implementing quality improvements.Job Class and Reports To:The Nurse Practitioner position will be a full time, non-exempt position that reports to the Lead Nurse Practitioner and Doctor/Owner at Hydration Room.Duties/ Responsibilities:Providing initial good faith exam for first time patients via phone/telehealth.Will need to complete telehealth appointments while in the clinic or office.Remotely triage, and treat patients via our online telehealth platform, including: proactively providing medical and wellness education, treating patients via our online chat-based telemedicine platform, recommending suitable treatment plans and assisting in care coordination and onsite visit planning for wellness member visits.Giving and receiving regular feedback on individual member cases.Providing a positive member experience in telemedicine interactions, including quality of care recommended and tone / messaging with which care is delivered.Function as day-to-day clinical provider, providing decision support, directing the multidisciplinary team.Coordinate with the physicians across the continuum of care, including, Doctor/Owner, Regional Clinic Manager, District Lead Nurses and Nurses.Participate in the provider schedule to provide coverage for our 7 day a week clinic schedule. This will require weekend availability on a recurring basis. Expected to work 3-4 weekend shifts per month.Performs other related duties as assigned and required.
