We are looking for an experienced Nurse Practitioner for position involves treating patients at skilled nursing facilities, assisted living and outpatient office. The nature of the position allows the Nurse Practitioner the opportunity to apply his/her medical training across a broad spectrum of clinical areas. Nurse practitioner get opportunity to further broadened the knowledge from rounding physician. The Nurse Practitioner (NP) functions: The Nurse Practitioner coordinates appropriate, timely patient care for a defined patient population(s), focusing on identified expected optimal outcomes individualized to the patient. The Nurse Practitioner Evaluates patients and order necessary labs, diagnostics and medication. Review labs, develop plan of care, collaborate with interdisciplinary team, management and physicians to ensure quality outcomes, and reduce hospital admissions. The Nurse Practitioner willing to travel different locations to see patients. Candidate Requirements : Previous clinical experience with Adult Medicine/Family Medicine/Geriatric considered a plus. Licensed and clean record with Illinois Department of Financial & Professional Regulation. Competent in computer skills including experience with EHR, proficient typing skills. Excellent organizational skills. High degree of ownership when caring for our patients.
