Community Care Network is looking for an Advance Practice Registered Nurse to join our team in Rutland, Vermont! Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals and families through responsive, innovative and collaborative human service About the Role: The Advanced Practice Registered works with a team of professional social workers, clinicians, case managers and others to ensure integrated care for adults experiencing serious mental illness. As a member of the client care team, the APRN will establish therapeutic relationships in order to facilitate client progress with treatment goals. To be effective, the APRN will have the demonstrated ability to balance independent clinical decision making with appropriate consultation from providers. Qualifications: Masterxe2x80x99s Degree in Nursing along with the appropriate Nurse Practitioner certification required. Unrestricted, current and complete State of Vermont APRN license required (or ability to attain upon employment). Experience working with adults with severe and persistent mental illness, substance use disorders, developmental disabilities, and co-occurring developmental disabilities and/or medical conditions required. Demonstrated current knowledge of psychiatric medications. Solid computer skills and familiarity with operating an Electronic Medical Record required. Ability to demonstrate compassion, sensitivity, and reasoning skills. Must have a valid driverxe2x80x99s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont. *Successful completion of a criminal history background check after hire.
