Provide primary care to the homebound/home-limited Clay and West Duval county patient population in their home. The nurse practitioner will be responsible for caring for the patient, ordering tests and medications as appropriate for each patient. Maintaining the medical records of each patient seen in an accurate and timely manner. Billing visits per Medicare guidelines and scheduling appropriate follow up appointments as medically necessary for each patient. A successful candidate will provide each patient with compassionate and responsive care and will communicate with the healthcare team internally and externally to help patients meet optimal health status and prevent hospitalizations.
