Job Overview:We are seeking an experienced and dedicated Advanced Practice Registered Nurse to join our healthcare team. As an Advanced Practice Registered Nurse, you will play a crucial role in providing high-quality patient care and ensuring the well-being of our patients. This position requires strong clinical skills, excellent communication abilities, and a passion for delivering compassionate healthcare. Responsibilities:- Conduct comprehensive patient assessments and develop individualized care plans- Perform utilization review to ensure appropriate and efficient use of healthcare resources- Administer medications and monitor patients for adverse reactions- Provide education and support to patients and their families on disease management, Chronic Care Management, HEDIS measures, treatment options, and preventive care- Coordinate with other healthcare professionals to ensure continuity of care- Assist with durable medical equipment (DME) ordering and management- Assist with Wound Care, Stitch removal, IVs- Perform venipuncture and collect laboratory specimens as required Requirements:- Valid Advanced Practice Registered Nurse license in Florida- Master’s degree in Nursing or related field- Certification as an Advanced Practice Registered Nurse (APRN)- Minimum 1 years of experience in a similar role- Proficient in utilizing electronic medical record systems, eClinicalWorks a plus- Strong knowledge of medical terminology, procedures, MRA documentation, HEDIS measures, and best practices- Excellent clinical assessment and diagnostic skills- Ability to work independently and collaboratively within a multidisciplinary team- Exceptional communication and interpersonal skills, and a Team player.
