As our Nurse Practitioner, you will play a vital role in delivering patient-centered care to residents within skilled nursing facilities. Your primary focus will be to establish a strong patient-provider relationship, assess patients’ health conditions, and implement remote monitoring solutions to enhance their overall well-being. You will collaborate with a multidisciplinary team to assess, plan, and implement care strategies to enhance the quality of life for our patients. This role requires a dynamic individual with strong clinical skills, excellent communication, and a passion for leveraging technology to improve patient outcomes. Key Responsibilities: Patient Evaluation: Perform comprehensive health assessments, including physical examinations and medical history reviews. Develop and implement individualized care plans based on residents’ needs and conditions. Establish Patient Relationships: Build strong rapport with patients, families, and facility staff to ensure collaborative and patient-centered care. Educate patients on the benefits of remote monitoring and address any concerns or questions. Collaboration with Facility Staff: Work closely with facility healthcare teams to coordinate care plans and implement remote monitoring strategies. Provide support and training to facility staff on the use of monitoring devices and data interpretation. Documentation and Reporting: Maintain accurate and up-to-date patient records using electronic health records (EHR) systems. Document assessments, care plans, interventions, and progress notes in a timely and thorough manner. Continuous Improvement: Stay current with advancements in remote monitoring technology and contribute to the enhancement of program protocols. Provide feedback on program effectiveness and suggest improvements for better patient outcomes. Qualifications: Ability to thrive in a fast-paced environment with a startup mentality. Ability to travel within the assigned territory- CLAY COUNTY and surrounding area Previous experience working as a Nurse Practitioner (NP) with licensure. Preferred experience with Electronic Health Records (EHR), e.g., eClinicalworks. Exceptional critical and analytical thinking skills. Strong verbal and written communication skills. Familiarity with medical terminology and clinical decision support. Preferred experience using GSuite: Gmail, Docs, Sheets, Slides. Team player with a positive attitude. Proficiency in listening, comprehending, and processing detailed information. Commitment to continuous learning and clinical improvement.
