The Full Time Nurse Practitioner (NP) will be responsible for acute and chronic disease management for assisted living and senior housing residents. The Nurse Practitioner (NP) will conduct a one time annual insurance assessmentand partner with the PCP and long term care staff to provide primary and preventative care while proactively identifying triggers for acute illness and chronic conditions. Depending on acuity the NPwill perform 5-7 assessments per day with a goal to improve the overall quality of care and decrease the need for avoidable hospitalizations.Theassessment will be conducted on Medicare Advantage plan members residing in long term care facilities in Tacoma, WA and the surroundingarea. The NPis typically scheduled in one facility per shift.
