The successful candidate will have a strong background in corrections medicine, chronic conditions, comorbidities, and acute care. This role is ideal for an advanced practice provider who is ready to take on a challenging and rewarding role in a fast-paced environment.Responsibilities:1. Provide comprehensive and personalized care to patients, focusing on the management of chronic conditions and comorbidities.2. Collaborate with other healthcare professionals to develop and implement effective patient care plans.3. Conduct physical examinations, interpret lab results, and diagnose health issues.4. Prescribe and administer treatment and medication as necessary.5. Monitor patient progress and make necessary adjustments to the care plan.6. Provide acute care services, including emergency treatment, to patients as required.7. Educate patients and their families on disease prevention and health maintenance.8. Maintain accurate and detailed patient records in compliance with privacy regulations.9. Stay updated on the latest medical research and apply this knowledge to patient care.10. Provide support and guidance to nursing staff, contributing to a positive work environment.Qualifications:1. Must be a certified Nurse Practitioner with a valid license to practice.2. Minimum of 3 years of experience in a similar role.3. Extensive experience in corrections medicine, chronic conditions, comorbidities, and acute care.4. Proven ability to work as part of a team and collaborate effectively with other healthcare professionals.5. Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families.6. Strong problem-solving skills and the ability to make critical decisions under pressure.7. Demonstrated commitment to continuous learning and professional development.8. Proficiency in electronic medical records systems and other relevant computer applications.9. One weekend/month is required.10.
