The Role We are looking for a Nurse Practitioner to manage chronically-ill patients in an outpatient setting. The NP will be responsible for supervising the implementation of the care plans. The position offers flexibility of schedule. Our company prides itself on the high quality of care and innovation of delivery. We have strong support and organizational team. Our main area of need for this position is Sugarland, Pasadena, Pearland, Katy, Central Houston Essential duties and responsibilities Our usual working hours are 8am to 4pm Monday to Friday Paid directly based on productivity, adjusted to your preferred schedule Routine schedule, with flexibility for acute issues in your area of coverage. Ensure clinical excellence and compliance with regulatory organizations and guidelines Proper documentation and charting skills Experience with EMR necessary Proficiency with eClinicalWorks EMR preferred Identify diagnoses to be used in care management and active medical management Reviews and manages medical conditions using clinical knowledge and judgment Manage follow up consultations and care coordination in-between visits About You: Ambitious and maintain a growth mentality Strong problem-solving abilities Personable and able to work in a team setting Collaborate with coworkers, facility staff, and family members Able to establish self as trusted partner for doctors Reliable and have a strong work ethic Experienced in geriatric care and/or preventative medicine (preferred).
