Manage the psychotropics of geriatric patients with behavioral health diagnoses commonly seen in primary care and long-term care settings- consistent with the scope of practice as either a family nurse practitioner or an adult-geriatric advance practice nurse. Collaborate with clinical psychologist and psychiatrist to provide high quality, evidence-based care in assisted living facilities, group homes, and long-term care facilities. Requirements: xc2xb7 Must be a licensed nurse practitioner/nurse prescriber in the state of Wisconsin and eligible as a Medicare/Medicaid provider. xc2xb7 Experience in geriatrics, behavioral health or long-term care is preferred; however, we can offer training to the new Nurse Practitioner. Consultation is within scope of family practice. Responsibilities: xc2xb7 Provides psychiatric assessment, consultation and medication management for residents in long term care facilities, in collaboration with primary care physicians, psychologists and nursing home staff. xc2xb7 Attends multi-disciplinary treatment team meetings. xc2xb7 We are willing to provide training and supervision.
