Duties will include, but are not limited to, disease management, performing physical exams, recording patient medical histories, and creating patient care plans that are aligned with patientxe2x80x99s goals. This position involves daily travel. Essential Duties and Responsibilities: xe2x97x8f Provide extensive primary care to patients, with the goal of helping the patient to age in place as appropriate and to reduce both unplanned hospitalization, unnecessary emergency department visits and nursing home admission. xe2x97x8f Provide Advance Care Planning discussion to illicit patientxe2x80x99s state goals of xe2x80x9cWhat Matters Mostxe2x80x9d in initial assessment and follow up visits. xe2x97x8f Assist the practice and participates with the Practice Administrator and Home-Based Primary Care Physician in meeting safety, performance, and quality measures for a Home-Based Primary Care Practice. xe2x97x8f Work collaboratively with other Patriot Home Care Services lines and HBPC interdisciplinary team to achieve patient and model goals. xe2x97x8f Time management xe2x97x8f Triage incoming messages, completed within the same day as the message was received. xe2x97x8f Complete all progress notes within 24 hours of the visit. xe2x97x8f Perform physical exams and patient observations. xe2x97x8f Record patient medical histories and symptoms. xe2x97x8f Order, administer and analyze diagnostic tests. xe2x97x8f Evaluate responses to treatments and medications. xe2x97x8f Appropriately prescribing medicines within the scope of prescriptive authority including schedule II-controlled substances. xe2x97x8f Use of most appropriate and safest approaches to pain management using non-narcotic approaches first when indicated. xe2x97x8f Diagnose health issues. xe2x97x8f Detect changes in a patientxe2x80x99s health, change in condition, and change treatment plans as necessary. xe2x97x8f Create patient care plans and contribute to existing plans. Meet documentation standards for all visits, chronic care management, advanced care planning, prolonged services and evaluation and management codes. xe2x97x8f Work with the HBPC interdisciplinary team (IDT) to coordinate care plans and services. xe2x97x8f Monitor and operate medical equipment. xe2x97x8f Involve community resources as needed. xe2x97x8f Consult with healthcare professionals. xe2x97x8f Train patients and families on managing illnesses or injuries and disease prevention. xe2x97x8f Maintain quality, productive relations with other Patriot Home Care staff, and vendors. xe2x97x8f All other duties as assigned. xe2x97x8f Must have prescriptive authorization. xe2x97x8f APN authorization in the state where employed to practice in Family Health by the Board of Nurse Examiners xe2x97x8f Reliable transportation, xe2x97x8f Valid driverxe2x80x99s license with automobile insurance The Qualifications Education: Completed an accredited, Board-approved masterxe2x80x99s or postmasterxe2x80x99s Nurse Practitioner Program or other Board-approved program that awarded an advanced degree; or a course of study considered by the State Board to be equivalent to that required for certification at the time the course was completed. Credentials/Licensure: xc2xb7 RN licensure in State of Delaware xc2xb7 Certification from Nurse Practitioner Program xc2xb7 Valid driverxe2x80x99s license Experience: xc2xb7 3+ years of experience as a Nurse Practitioner preferably in a skilled home health, PACE or hospice setting xc2xb7 Proven experience providing care to an older adult/elderly population.
