Acute Hospital Care at Home (AHCaH) providers operate similarly to inpatient hospitalists with a more virtual care approach. Cooperating with other team members through huddles and as needed to assess patient needs and identify appropriate patients for this model of care.Job DutiesThrough reliable communication, remote patient monitoring and outlined protocols, AHCaH enables patients who meet hospital admission criteria to receive care in the comfort of their home.
