The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. HopeWest PACE participants receive in home care services and transportation to a HopeWest PACE center for primary medical care, social and recreational activities, and other senior care services.We understand the importance of a system that works together. Your expertise, drive and passion can help us carry out our mission to improve lives and advance healthcare.The Unique Responsibilities of the Nurse Practitioner at HopeWest PACE:Provides a wide range of Primary Care services including diagnosis and treatment of chronic and acute illness, preventative care, and urgent medical needs all in collaboration with the Chief Medical Director.Conducts health assessments and evaluates patient needs in the clinic, the PACE Day Center and the patientxe2x80x99s homeActively collaborates with the entire PACE interdisciplinary team for the highest quality care of each participant.Make appropriate and timely referrals to our contracted providers out in the communityWhat You Bring:Minimum 2 yearsxe2x80x99 (5 years preferred) APRN experience in a primary care clinic settingBoard Certified Advanced Practice Nurse licensed in the state of ColoradoMust have Prescriptive Authority in Colorado and current DEA licenseEligible or currently possesses Medicare/Medicaid UPIN numberValid Colorado driverxe2x80x99s license, adequate automobile insurance and access to reliable transportationAbility to work collaboratively and communicate effectivelyWhat We Offer:Competitive pay of $58-$60 per hour$5,000 sign on bonus4x10 hour shifts per week with on-call rotationCigna Health, Dental, and Vision insurance effective the month following start dateImmediately start accruing PTO and medical leave15% discount at Spoons Bistro & Bakery, and Heirlooms locationsUp to 3% match on 403B retirementEmployer paid life insurance and EAPMileage reimbursement for work-related drivingAdditional pay for bilingual fluencyTuition reimbursement programAt HopeWest, we hold strong core values that are followed internally with staff as well as in the daily care of our patients & their loved ones.
