We’re on the lookout for a skilled Nurse Practitioner specializing in wound care for our home health services. In this role, you’ll travel to patients’ homes across Northern Colorado to assess, treat, and manage complex wounds. You’ll collaborate with a dedicated team, providing education to patients and their families on wound care techniques.xc2xa0Day to Day Responsibilities:xc2xa01. Present a professional demeanor and maintain a positive attitude while strictly following policies and procedures.xefxbbxbf2. Conduct thorough health assessments, accurately documenting medical and psychosocial histories in electronic medical records during home visits.xefxbbxbf3. Document patient-reported symptoms with precision and attention to detail.xefxbbxbf4. Perform tailored physical examinations to evaluate wound conditions prevalent within our patient community.xefxbbxbf5. Utilize examination results and diagnostic tests to formulate accurate diagnoses.xefxbbxbf6. Apply advanced wound care techniques, including sharp debridement and the application of amniotic allografts as appropriate.xefxbbxbf7. Educate patients on preventive measures to mitigate disease risks and promote overall health restoration.xefxbbxbf8. Collaborate closely with fellow healthcare professionals, including physicians and nurses, to ensure the best possible outcomes for patients.xefxbbxbf9. Order and interpret laboratory and diagnostic tests with clinical expertise.xefxbbxbf10. Conduct comprehensive follow-ups with patients to evaluate the effectiveness of prescribed treatments.xefxbbxbf11. Contribute to the development of clinical and quality policies aimed at improving care delivery and patient outcomes.xefxbbxbf12. Fulfill any additional duties or responsibilities assigned by superiors.Job Must Haves:xc2xa0- Demonstrated experience as a Nurse Practitioner in diverse healthcare environments such as clinics, hospitals, or emergency rooms.xefxbbxbf- Ability to thrive in a fast-paced environment, demonstrating adaptability and resilience.xefxbbxbf- Up-to-date knowledge of health regulations and patient care standards.xefxbbxbf- Exceptional written and verbal communication skills to effectively engage with colleagues and patients.xefxbbxbf- Strong team player with a positive attitude and ability to foster camaraderie.xefxbbxbf- Proficiency in using Microsoft Office Suite and/or Google Suite to facilitate efficient workflow and communication.Awesome Bonus Skills:xc2xa0- High-energy individual with a strong team spirit, eager to contribute to the success of our growing practice.xefxbbxbf- Demonstrates unwavering compassion towards patients, ensuring they receive top-quality care.xefxbbxbf- Preferably, a minimum of two years of experience as a Nurse Practitioner, demonstrating a depth of clinical expertise.xefxbbxbfxefxbbxbf- Ideally, one to three years of specialized experience in wound care, including prior experience in debridement procedures.xefxbbxbfxefxbbxbf- Consistently demonstrates thoroughness and attention to detail in completing tasks.Education/Certification/License Requirements:- Valid Board Certification as a Nurse Practitioner in the state of Colorado.xefxbbxbf- Advanced Wound Care Certification is highly preferred.xefxbbxbf- Current Basic Life Support (BLS) Certification is required.Work Environment/Physical Requirements:- This role involves traveling between patient homes, requiring a valid state driver’s license and a clean driving record.xefxbbxbf- Ability to travel up to 70% of the time, including driving distances exceeding 50 miles.xefxbbxbf- Regular physical activities such as sitting, standing, walking, bending, and lifting objects weighing up to 50lbs.xefxbbxbf- Maintains a polished and professional appearance during all business interactions, adhering to a business casual dress code.
