DESCRIPTIONThe Nurse Practitioner will be responsible for managing the patientxe2x80x99s wound care and visit documentation in a timely and efficient manner, and working with nursing staff to develop plans of care to increase quality and control costs at our partner facilities.Responsibilities:xc2xb7 Provision of hands-on care to patientsxc2xb7 Debridement of wounds by sharp, enzymatic and autolytic methods, able to be completed safely.xc2xb7 Application and management of topical wound care and associated wound management products.xc2xb7 Direction and recommendation of associated services to improve wound healing outcomes. This includes but is not limited to labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.xc2xb7 Interpretation of diagnostic tests and recommendation of appropriate follow up to same.xc2xb7 Recommendations for prescription of required medications and/or Durable Medical Equipment or supplies (DME) to facilitate wound healing and/or wound prevention within her/his scope of practice, license and as per the collaborative agreement with the supervising physician.xc2xb7 Diabetic Foot Ulcer risk assessment for patients with diabetes, as per the ADA guidelines.xc2xb7 Education of patients as to the topical and systemic management of their wounds.xc2xb7 Education of the Primary Care Physician (PCP), CM, LTC staff or referring Physician on current wound management practices.xc2xb7 Provision of regular updates to the PCP, CM, or referring Physician on the status of their patientxe2x80x99s systemic conditions associated with wound care.Qualificationsxc2xb7 Nurse graduate of accredited school of Nursing and Nurse Practitioner programxc2xb7 Possesses and maintains Nurse Practitioner national certificationxc2xb7 Paid training and orientation provided. (Approximately 2 months)xc2xb7 Current, active, unrestricted Nurse Practitioner license in the state of Virginiaxc2xb7 Ability to lift 25 lbs. or morexc2xb7 Must Have full range of mobilityxc2xb7 Must be able to reposition patientsxc2xb7 Valid and unrestricted Virginia driver’s licensexc2xb7 Excellent verbal and written communication skills in a professional environmentxc2xb7 Ability to set priorities while managing multiple tasksxc2xb7 Detail oriented with excellent organizational skillsxc2xb7 Person of high character and integrity requiredJob Type: Full-Time and Part-Time positions available (2 or 3 days/week)LOCATION: Charlottesville, Virginia (and surrounding areas)
